Services: A Camden County health program that offers room, board and personal care services to physically impaired individuals who are seeking an alternative to institutionalization. Participants are placed in the home of carefully screened, certified home health aides who provide supervision and assistance with activities of daily living. Sliding fee scale: Program subsidy is available to qualified individuals.

Services: This program provides a supplement to the assistance an individual receives from his/her natural support network of family, friends, and neighbors as well as from Community Agencies and volunteer groups. CAP provides Case Management, Respite Care, Homemaker Services, Environmental Accessibility Adaptations, Personal Emergency Response Systems, Home Delivered Meal Service, Caregiver/Recipient Training, Social Adult Day Care, Special Medical Equipment and Supplies, Chore Services, Transportation, Attendant Care, and Home-based Supportive Care. 

Eligibility: 65 or older or 21-64 and on Social Security Disability; resides in a unlicensed residence; has received a pre-admission screening and has been determined to require Nursing Facility level of care; qualifies for SSI in the Community or qualifies for Medicaid only; qualifies for New Jersey Care. Jersey Assistance for Community Caregiving (JACC)

Services: Provides in-home services to seniors at risk of placement in a nursing facility. JACC includes an array of services designed to supplement assistance given by the individual’s caregiver network of family, friends, and neighbors. JACC works to enhance the individual’s community care options by designing service plans uniquely tailored to the individual. JACC also strengthens the ability of caregivers to continue in their vital role as primary support providers. 

Eligibility: US Citizen or qualified alien and a New Jersey resident 60 years or older who requires a nursing facility level of care but wishes to remain at home but is not financially eligible for Medicaid or Medicaid Waiver Services. Does not have an alternate means to obtain the needed support. Resources cannot exceed $40,00 for an individual, or $60,000 for a couple; is willing and able to share in the cost of services. Participants share in the cost of their care on a sliding scale based on their income.

Services: Offers relief to those families experiencing fatigue and stress because of the physical and emotional demands of caring for the elderly or disabled family member 18+ years of age. Types of service include home health aides, adult day care, live-ins, temporary in-patient care in a licensed facility and case management. respite care is available for a few hours per week to much more including weekends or vacation coverage.

Hearing Aid Assistance to Aged and Disabled 

HAAD 

Eligibility: 65+ or between 18-64 receiving Social Security Disability and income for 2024 is no greater than $52,142 single or no greater than $59,209 if married 

  • • HAAAD provides up to a $500 reimbursement to offset the purchase of a hearing aid; or up to a $1,000 reimbursement to offset the purchase of two hearing aids. 
  • • If one hearing aid is purchased one year, and another hearing aid, for a different ear, is purchased the following year, individuals can reapply for reimbursement of the second hearing aid. This benefit can only be received once every three years from the date of purchase of each hearing aid. 
  • • “Hearing Aid” means a custom-fitted ear-level or body-worn electronic device which enhances communication for the hearing impaired. 
  • • Beneficiaries must be currently enrolled in the Pharmaceutical Assistance to the Aged and Disabled (PAAD) program or Lifeline Utility Assistance (Lifeline) program, or. 

Meet the PAAD and Lifeline eligibility criteria described below. 

If you are currently enrolled in the PAAD or Lifeline program, you must complete a HAAAD application (Word) (PDF) (Word Spanish) (PDF Spanish) and submit the following documentation: 

  • • A receipt for the purchase of your hearing aid. 
  • • A written statement from your physician attesting to the medical necessity for obtaining a hearing aid.
