FamilyCare/KidCare

  • Provides coverage to families in need of health insurance who do not qualify for TANF benefits.
  • Children up to age 21 are eligible, as are parents or caregivers of children up to age 19.
  • Services include well child visits, prescriptions, doctor's visits, vision care, laboratory services, preventive services and hospitalization.
  • Eligible participants can choose an HMO provider to manage their healthcare needs.

  • Serves the aged (over 65), blind and disabled who live in the community or an eligible nursing home but who do not qualify for SSI.
  • Provides full Medicaid benefits.

  • Individuals aged 65 or over, blind or disabled living in the community or an eligible nursing home may qualify if they meet low income and resource levels.

  • Provides limited home and community-based services as an alternative to institutional care to elderly or disabled individuals choosing to remain in the community.
  • Eligible individuals need to have medical insurance.

  • Community Program:
    • Assists individuals 65 or older, blind, disabled or pregnant.
    • Provides eligible individuals with a variety of medical services including physician care, dental care and access to independent clinics.
  • Medically Needy Nursing Home:
    • Client's income must be less than the nursing home reimbursement rate plus maximum monthly income.

  • Serves individuals 65 or older, blind or disabled.
  • Provides full Medicaid benefits. Does not include nursing facility care or a personal care assistant.
  • Provides care at a Comprehensive Personal Care Home, an Assisted Living Residence or in the home of an unrelated person. A package of assisted living services is available to each participant.

  • Serves blind, disabled children and adults in need of institutional care but who choose to remain in the community.
  • Individuals must meet, at the minimum, the nursing home level of care criteria.
  • Provides full Medicaid benefits.

  • Provides ambulatory prenatal care services.
AIDS Community Care Alternative Program (ACCAP) >> Top (phone numbers) <<
  • Serves individuals diagnosed with AIDS and needing nursing home care who choose to remain in the community.
  • Provides full Medicaid benefits and additional nursing services.

  • Assists terminally ill individuals living in the community or in a nursing home.
  • Clients must be eligible for Medicaid Only, New Jersey Care, the Waiver Programs, or AFDC-related Medicaid.
  • Services include nursing care, supervisory physician services and durable equipment and supplies.

  • Helps individuals 18 to 65 years of age disabled by traumatic brain injury.
  • Provides full Medicaid benefits with the exception of nursing facility care.
  • Eligible persons receive a variety of services through the assistance of a case manager.
